Grounded in our person-centered approach, our Family Navigator works one-on-one to create an action plan with your family. Together, we’ll envision, plan, and implement steps to ensure a rich everyday life and secure future for your family member with disabilities.

Structure, continuity, and clarity for your family
Consultation may be done in-person, by phone, or on Zoom, based on the preferences and location of the family. Some families only need a brief consultation to move through a situation, solve a short-term problem or gain access to resources. Other families may prefer an on-going process. We are available for both!
Our Family Navigator provides structure, continuity, clarity, and resources so that families and individuals can move forward on their own journey to their person-centered goals.
We can help families in areas that include, but are not limited to:
Creating a full picture for a meaningful, everyday life
Identifying and prioritizing next steps towards Independence
Developing friendships and community connections
Navigating Waiver/Public Support systems to best meet the individual’s needs
Creating, consulting, and/or ongoing facilitating of Personal Support Networks
Identifying opportunities for contribution
Planning for housing
Long-term Planning for stability and security
Connecting to resources for programming, supports, services and specialized expertise
